Abbotsford Community Hub Centre (ACHC)
ACHC providing a range of support services across Fraser East BC communities including, Abbotsford, Mission, Chilliwack and all the way to Boston Bar.
In April 2020, Phoenix Society merged with the Positive Living Fraser Valley Society (PLFV), based out of Abbotsford. The decision to bring our organizations together was a simple one; Partnerships are critical in creating a community that supports and serves everyone. Phoenix Society has since become the anchor agency of the Abbotsford Community Hub Centre which opened its doors in late 2019, providing a wrap-around model of care for those experiencing health, and social challenges.
HIV Outreach Services : Monday through Friday 830am-430pm, not including stats
Our HIV Outreach workers provide one-on-one case management for diagnosed HIV+ positive clients, living in the Fraser east region, who require additional supports. Supports provided:
- Support with transportation to medical appointments, to foster understanding and ensure ongoing attendance is completed
- provide emotion support while navigating the medical field
- Assistance with basic needs, that improve an individual’s quality of life
- Medication reminders & check-ins to ensure health and safety of client’s wellbeing
- Managing client appointments & ensuring positive relationships/experiences are kept in the community by helping client keep appointment dates, and/or rescheduling dates when clients are unable to make set times
- Access to a high protein food bank, monthly as a satellite to the Archway Food Bank
Prevention Assessment Referral Clinic (PARC) : Daily 8am -530pm, Including stats
Provides a drop-in Supervised Consumption Site for substance users. This clinic offers services to vulnerable populations in Fraser East region of British Columbia, providing assessments, referrals, harm reduction supplies. PARC averages approximately 9000 visits per quarter.
Services include:
- Daily lunches starting at 11 am, a cup of coffee between 8am -11am (first come, first serve)
- Access to shower and laundry facilities Monday- Friday, subject to change without notice
- Access to a Fraser Health on-site nurse practitioner when permitted or an appointment is made during week days
- Access to naloxone training, naloxone kits and other harm reduction supplies
- Access to peer one on one support
Prevention Alliance Integration, and Community Education (PAICE): Monday – Friday 830am-430pm, not including stats
Provides support to those who have been diagnoses with HIV, Hepatitis or other related STBBI’s to increase their knowledge in evidence-based prevention measure. This program also provides trainings to those in community or other services providers on HIV, Hepatitis and STI 101. Peer support groups for those diagnosed are available monthly, with the addition of other activities planned through out the month. Point of care HIV testing is also available on site.

Fraser Health
- Sexual health clinic: Provides free and confidential testing, counselling, and education about sexually transmitted infections, HIV, and AIDS. To book an appointment, please call #604-702-4906
- Interagency Care Team: Consists of a social worker and nurse, who provides support and case management to individuals in community who struggle with being connected to services to improve quality of life. This is a temporary service the support connection to those who may be living with Physical challenges, mental health, and or addiction challenges
- Nurse Practitioner’s: Primary care is provided through referral or appointment only. This service is not considered a walk-in clinic.
Hub Pharmacy: Provides Pharmacy services, to anyone in community

The Ministry of Social Development and Poverty Reduction: Provides an onsite integration specialist on a drop-in basis to support those who have challenges accessing the ministry office directly. For information on times and dates available contact #604-854-1101
