HIV and sexually transmitted and blood-borne infection support

Phoenix’s HIV outreach team offers compassionate, low-barrier, one-on-one case management to people at risk of or living with HIV and other sexually transmitted and blood-borne infections (STBBI). Our services include transportation to medical appointments, medication check-ins, emotional support to navigate the health care system, and assistance with daily needs that affect overall wellness. 

• Open to anyone in the community who is living with or at risk of a blood-borne infection
• Referrals can be made by self, care teams, health providers, or community supports 
• Phoenix program participants can also connect through their case manager or on-site medical clinic
