Phoenix Pathways

Phoenix Pathways provides a continuum of housing options in Chilliwack, including a 42-bed long-stay shelter, 27 supportive housing units, and 22 enhanced health care units supported by the Fraser Health Enhanced Care team. Residents have access to 24/7 wraparound services, including culturally safe programming, case management, meals, laundry facilities, and support to maintain housing stability.  

About

Supportive housing and enhanced care units are self-contained, each with a kitchen, shower and bathroom. Enhanced care housing offers enhanced health services for adults facing overlapping mental health, substance use, and medical needs, including medication support and daily living assistance. The long-stay shelter allows residents to stay as long as needed, while they work with case managers to transition into more permanent housing. The shelter offers shared rooms with four beds each.

 

Individual and group supports are available to help residents connect with community resources, pursue education, employment, and health goals, and move toward greater independence.  

How to Particpate

Interested applicants can connect with a community partner to complete a Supportive Housing Registry application and submit it to the registry. When a vacancy occurs, criteria for the vacancy is sent to all community partners. Community partners will submit Supportive Housing Registry applications along with a VAT (Vulnerability Assessment Tool) to the Access and Assessment Coordinator from BC Housing and Program Manager of Rowat. The applications are brought forward to the Community Access Table and assessed based on need and criteria. Once the appropriate applicant is decided, The Program Manager and BC Housing Access and Assessment Coordinator will schedule an interview to determine program suitability.

What to expect

Once enrolled, an interview will be scheduled with housing staff who will provide clear guidance on what to bring and offer personalized support throughout the admission process. The team is dedicated to making the experience as smooth as possible and is always available to answer questions or provide information.
