Justice: Community Residential Facilities (CRF) and Transitional Housing

Phoenix’s Rising Sun facility hosts several Community Residential Facilities (CRF) programs in partnership with Correctional Services Canada (CSC) to support individuals transitioning from correctional facilities. These programs provide structured, community-based living for individuals involved with the criminal justice system. Where appropriate, our staff support community reintegration by fostering participants’ stability, accountability, independence, and meaningful connection within the community. 

Community Residential Facilities (CRF)

Phoenix’s Community Residential Facility (CRF) provides structured, community-based living with 24/7 on-site support. The program offers stability, accountability, and meaningful opportunities for growth. 

 

Each resident is matched to the level of supervision and care that reflects their individual needs. Our team includes case managers and trained support staff who provide daily guidance, crisis support, and individualized case planning. Staff receive specialized crisis and risk management training in partnership with CSC. 

 

Some residents benefit from enhanced daily support, while others move toward greater independence, which can include working, reconnecting with family, and building life skills. 

 

The CRF program is about safe, supported connection to community, strengthening individuals and the community together. 

Aging Adults

Phoenix’s Aging Offender Program provides specialized, supported living for older adults involved with CSC who are living with complex health needs. Aging Adults have access to 24/7 medical and support staff, including nurses, health care assistants, and case managers. Phoenix staff support individuals with medical appointments, medication reminders, and daily living needs.  

 

The program is open to individuals aged 55 and older, as well as younger adults with significant health concerns. There is no prescribed length of stay. If someone’s care needs change, we work collaboratively to ensure a thoughtful transition to the most appropriate setting. 

 

Program participants also have access to in-house recreation, music therapy, game nights, and structured activities. Combined with programming offered in partnership with CSC, this program helps foster community, purpose, and well-being. 

Transitional Housing for Those Exiting the Justice System

Phoenix’s Transitional Housing for individuals exiting the criminal justice system offers the next step toward independent community living. This program is designed for those who have demonstrated readiness for increased responsibility and are nearing the completion of their sentence. 

 

Residents live in private, self-contained suites, within a structured, substance-free environment. A dedicated team is available 24 hours a day to provide guidance, accountability, and encouragement. Phoenix staff support residents in preparing for and attending meetings with CSC and parole officers, while offering regular check-ins to promote stability and progress. 

 

Supports include helping participants develop life skills, obtain appropriate ID, plan for and seek long-term housing elsewhere and access stable health care. Residents can also pursue education and employment while engaging in social and recreational activities.   

 

Residents build their confidence, strengthen their independence, and create a solid foundation for long-term wellness and community connection.
