Abbotsford Community Hub Centre

Partnerships are critical in creating a community that supports and serves everyone. To this end, Phoenix Society has become the anchor agency of the Abbotsford Community Hub Centre which opened its doors in early 2019, providing a wrap-around model of care for those experiencing health and social challenges.

Phoenix provides the following services through the Hub:

  • HIV outreach services for HIV+ individuals throughout Fraser East, many of whom are housing insecure and battling with addiction and mental health issues
  • Mobile outreach harm reduction services to Abbotsford and Mission, also working with other Programs in Chilliwack to ensure adequate supplies and overdose prevention education and kits are available
  • Drop-in services, including daily lunches, a food bank for those living with HIV or viral hepatitis, shower and laundry facilities, assessment, referrals, access to on site medical care, harm reduction supplies, peer counselling, onsite supervised consumption services and connections to shelters

Onsite Partners

  • Fraser Health provides a Sexual Wellness Team, Mental Health and Substance Use team, and Nurse Practitioner services
  • An onsite pharmacy provides OAT and other addiction services
  • The Ministry of Economic Security and Poverty Reduction supports an onsite financial aid worker
  • Inasmuch Community Society provides onsite settlement services for refugee claimants
  • The Abbotsford Association for Healthy Aging coordinates and promote activities supporting the health and well-being of seniors in the Abbotsford area
